Retrospective notice. This incident occurred on 24 August 2026 from 12:52 to 14:58 BST and is being recorded here after the event. A fault in our internal authentication service, following an automatic database failover, caused a proportion of calls to POST /donors and donors/match to return HTTP 500. Failed requests were rejected before any data was written. Service was restored at 14:58 BST by restarting the affected service. We have added alerting and are making changes to prevent recurrence.
