Retrospective notice. This incident occurred on 30 August 2026 from 19:39 BST to 00:22 BST on 31 August and is being recorded here after the event. The same internal authentication fault as on 24 August recurred and caused intermittent HTTP 500 responses on POST /donors (about 3% of calls) and donors/match (about 1%). Failed requests were rejected before any data was written. Service recovered without data loss. A permanent fix is in progress.
